FACILITY USE REQUEST

GRAND RAPIDS ALLIANCE CHURCH 1705 SE 7th Ave, Grand Rapids, MN 55744

Use this form for ALL building use reservation requests. Requests should be made as far in advance as possible as the facility is reserved often. Plans should not be made until you have received confirmation from the church office. Allow 10 days for the request to be processed.

Custodial services are required for ALL reservations over 10 people.

GENERAL RULES THAT APPLY TO ALL FACILITY USE

  1. No smoking, no alcoholic beverages and no gambling in any part of the church facility.

  2. GRAC will not be responsible for any lost or stolen items.

  3. Any property damage or loss of property must be reported and may be subject to replacement cost by the user.

  4. No food or beverages are allowed in the main sanctuary.

  5. Rooms that are NOT reserved shall be off limits and kept available for other usage.

  6. Children shall not be left unattended. An adult must be available for child supervision at all times,

  7. Use of decorations shall be limited and solely the responsibility of the reserving party for clean up. This includes, but is not limited to, signs, paper, rice, bird seed, glitter, streamers, flowers, etc.

  8. Person signing this for shall be fully responsible for restoring area and furniture to original locations.

  9. Person signing this form shall be fully responsible for removing all items associated and for general clean up.

  10. Youth Room - DO NOT move any equipment on the stage. Contact the Youth Pastor if space is needed on the stage.
